Dear Client,

In order to keep up with the current bureaucracy, created by the new Personal Information Protection Act of B.C., we are required to supply you with the following information.

Personal Information Policy - Types of Information We Collect
TLC Teresia LaRocque Coaching & Associates collects certain personal, and confidential information about you. That information is provided by you or is obtained via communication between yourself and the Associate Coach you are working with.

Examples of sources from which we collect information include:

• Interviews and phone calls with you
• Questionnaires completed by you
• Face to face meetings with you
• Letters or emails from you
• Notes taken by the Associate Coach during your meetings
• Notes and/or recording during group teleconferences (if applicable)

Privacy Policy
All information gathered by TLC Teresia LaRocque Coaching & Associates is 100% confidential and shall not be disseminated in any way, shape or form. The policy of TLC Teresia LaRocque Coaching & Associates is to never disclose names or to identify a person or their company in anyway when discussing concepts or sharing ideas.

TLC Teresia LaRocque Coaching & Associates values your relationship as a client and will do everything to keep that trust by keeping your confidential information between you and the Associate Coach you are working with. Some of the information discussed with the Associate Coach may be of a very sensitive and personal nature. This information will be kept confidential between yourself and the Associate Coach and any paperwork will be kept in the restricted office and computers of the Associate Coach and/or her assistant, Donna Paulson.

Use of Personal Information
TLC Teresia LaRocque Coaching & Associates uses the information under this policy to provide you with the services agreed upon in your agreement.

Parties to Whom We Disclose Information
As a general rule, TCL Teresia LaRocque Coaching & Associates does not disclose personal information about our clients or former clients to anyone. However, to the extent permitted by law, certain non-public information about you may be disclosed in the following situations:

• To comply with a validly issued and enforceable subpoena or summons
• Group Teleconferences are recorded and distributed to the participants of the group in which participants agree to a confidentiality statement
• Transactional information sent to our Bookkeeper and/or Accountant

Confidentiality and Security of Personal Information
Any personal information about you is kept in the offices and computers of the Associate Coach and/or her assistant Donna Paulson. Employees of and contractors to TLC Teresia LaRocque Coaching & Associates understand and agree to the policies of our company regarding non-disclosure agreements.
